Treatment planning : All-on-4 has been developed for Immediate Function.
Critical criteria for this to be possible are:
Possibility of achieving primary implant stability (> 35 Ncm insertion torque)
No extensions over one-tooth each side
No severe para functions
If there are extraction sites, make sure they are thoroughly cleaned. It is advisable to place implants between extraction holes.
Normal pre-treatment examination and evaluation of the patient, including radiographic imaging
Possibility of placing tilted implants posterially (3045°).
If the angulation is 30° or more, it is necessary to splint the tilted implants.
By tilting the posterior implants, the distal screw access holes should normally be planned to be located at the level of the second pre-molar.
The all-acrylic bridge is typically designed to have maximum twelve teeth and should normally not have more than one-tooth cantilevers on each side.
Prior to treatment, it is advisable to make a model of the opposite jaw, determine the optimal vertical height, as well as decide shade and shape for the artificial teeth.
Step-by-step : The following step-by-step descriptions outline the main procedures for treating totally edentulous jaws using the
All-on-4 concept. The concept can be carried out using both Brånemark System
® and Replace®Select implants.
